SHEET SUMMARY REPORT - "columns to display" not showing up


I followed the video as was trying to create a simple SHEET SUMMARY REPORT.  I've created 3 sheets under folder "Sample Q3 Priorities" folder, when it's time to select the "columns to display" (ie. task name, % completed, status, etc are not showing up), the column names are not showing up.

Each sheet template are identical. (Please see attached for the screenshot reference). 

Any idea why?
