Autopopulate Addresses

I am using Smartsheet Forms to evaluate building sites in the field. Can I associate each site with a specific address, which would then get autofilled after user chooses a site name from a dropdown list? I can provide the list and associated addresses (e.g., in a table), I just don't know if Smartsheet can autofill the address once site name is selected.

You can't have it autofill within the form itself, but you can reference a list to populate the sheet as each submission is made. Would that work for you?

Ok. Is the table going to be on the same sheet as the form or a different sheet?

Doesn't really matter. I have an excel file now, so uploading it as a separate sheet may be cleaner.

Ok. So if it is on a different sheet you will follow the proper steps for creating cross sheet references and you will want to use a formula similar to...

=INDEX({Table Sheet Address Column}, MATCH([Site Name Column]@row, {Table Sheet Site Name Column}, 0))

To add to Pauls excellent advice.

Another option could be to use the URL-query feature. You could then create forms for each site and then the values would be included automatically.

I just developed a solution for a client where we use and include an unique URL on each PDF so their customer can submit information easily.
