I'm new, help with dashboard widget, creating a widget to allow someone to submit 2 basic fields

Apologize in advance for my lack of knowledge...

Example:

I have a smart sheet already created. 2 columns, Company Name & Account Rep. Trying to create a dashboard/portal that will allow anyone to to just submit the Company Name and Account Rep name and have it automatically add to the sheet.

I’d recommend using a form instead. You can add the form to show inline in the Dashboard if you want to.
