Template for insurance claims

We have a company that needs track insurance claims inputted by various staff members. Is there a template that anyone has used before that may suit these requirements?

If I understand correctly, insurance claims will have to go through Approval Statues. This template can track these approval statues, as well as display all the metrics on a Dashboard.
