Form - Heading/Description & Dividers not copied from one to another

Anyone who can assist, greatly appreciate your time and assistance.

Created a Form within a Smartsheet, the Form includes Headings/Descriptions and Dividers. When the Smartsheet is copied as 'new' to use as a template and create another Form, the Headings/Descriptions and Dividers do not copy into the new Form. Is there a way to have this information copy into the new Form?

Thank you to anyone who may be able to provide insight into this.

When you save the sheet as new, make sure the box is checked like the screenshot below. Then the form should duplicate with the sheet.

Can you describe your process in more detail and maybe share the sheet(s)/copies of the sheet(s) or some screenshots? (Delete/replace any confidential/sensitive information before sharing) That would make it easier to help. (share too, andree@getdone.se)

Hi and thank you for your response! Good to know that the form should copy Headings/Descriptions and Dividers.

Unfortunately I am not able to share the Smartsheet/Form due to information contained, and if I change the data it will affect those using the Smartsheet/Form/Survey currently...let me attempt to explain.

Smartsheet created, Form created and in use as a survey importing data to the Smartsheet

Attempting to create another using the original Smartsheet as a 'template': Copied Smartsheet as 'new', select 'new' Form, column titles appear in the new Form however headings/descriptions and dividers do not.

I hope this information provides an explanation of what I am attempting to do.....if not, please let me know. I really appreciate your assistance!

Yes, new doesn't retain the information.

Try and duplicate the form instead. That should work.

I was able to duplicate the form, however when I bring up the 'new' Smartsheet and select Forms/Manage Forms, the duplicate does not appear in the selection as a choice.

Thank you! I had copied the Smartsheet from the file menu (does that make sense?), rather than file, save as New.

Copying for Smartsheet the way you explained, it worked!

Amazes me how Smartsheet 'knows' which Form to use for … which Smartsheet and how they are innertwined

Each sheet has a unique URL and so does each form (one of the perks to being a web based platform). This means that they only have to link the two URL's together.
