Is anyone else seeing issues with Multi Select option in a form?

Hi, I am trying to create a multi select field in a form but when I use the option, the tool is still forcing single select. What am I missing?

What do you see if you click on the dropdown in the form?

Can you maybe share some screenshots? (Delete/replace any confidential/sensitive information before sharing) That would make it easier to help. (share too, andree@workbold.com)
