Agree to terms before submitting a form

I'm creating a form where the requestor will need to read a document and then "agree" to the terms in the document before submitting their request. How do I do this?

I would recommend linking to the document and storing the document outside of the form.

You can do conditional logic in forms now!.

You'll have to add a checkbox column in your Smartsheet called Agree... then build out your form with the Agree column first. Then if the user checks agree, reveal the rest of your form.

Thanks. How do I attach a document to the form that they have to read before clicking agree?
