Using form to update dates on already created row

Hello,

I am building an automated training record tracker in Smartsheets. Currently, we have a form employees complete for recurring annual training they participate in which includes date of training, rating, and feedback. Instead of a new line being created for each form submission each year is there a way for the new form submission to replace the data in the currently created row? Any help would be appreciated.
