IFERROR INDEX MATCH Returning Duplicates

It seems as though my formula is returning multiples but only on one record (where it pulled it over fifteen times). Any idea what can be causing this?

    Hope you are fine, could you please add a copy or screenshot contains columns headers and a sample of what the formula return and the reference sheet the you collect the data from using index formula.
