Form submissions

Is there a reason that the form keeps erroring out after doing submissions one after the other? When it errors out, it won't allow you to submit the information so you have to refresh the form and then re-type all the information again?

There are some things to test. Start with these.

• Log out / in
• Test it in another browser
• Test in private/incognito mode
