"Created By" Field Inconsistent

I have a sheet that is populated by two forms. When a row is added from one of the forms, the Created By field is populated with the user's email address that sent the form, but the other form just populates as "web-form". Both of these forms are accessed from a single dashboard, so users are always signed in when submitting them. I tested them myself and I cannot get both forms to populate my email address. One form always includes it while the other never does. Is there some condition that would cause this behavior?

This is just a shot in the dark but in a form's settings, there is an option under security which requires people to log in to fill out form. Could this option be check for one of the forms but not the other?

NOTE: If there's a Created by system column in the sheet, and only registered Smartsheet users can submit the form, the column reflects the email addresses of the users who submit the form. When the form is accessible by anyone with the link, the Created by column shows web-form@smartsheet.com. (For information about System columns, see Use a System Column to Automatically Add Information to a Row.)
