Hello:
Can I add a formula to a field that activates as they use the form. Such as a vlookup that has the name of a supervisor and then looks up (vlookup) to pre-populates the contact info (email and phone) from a smartsheets form.

Forms can't contain formulas. You can however have columns in the sheet that are column formulas that contain the VLOOKUP. So once the information is entered the VLOOKUP is inserted into the row automatically. Assuming all the supervisors are listed in a sheet somewhere this would work.
If on occasion that information needs to be entered you can use logic in the form to have a dropdown that lists all the supervisors and if they select Other it gives them two more options to fill out and than have the VLOOKUP column start with an IF statement looking to see if those two entered information columns are blank or not.
Tip: Create column formulas by right-clicking on the formula in a cell and selecting "Convert to Column Formula"
