Check-box not working

I set one of my columns as a checkbox so the information can be marked as complete once it is reviewed. However, the small checkbox is not populating the column entries. I have set this up before on another sheet with no issue, so not sure what is going on. I have unchecked the checkbox option and selected it again several times with no luck. I also tried creating a new column for the checkbox, still no luck. Ideas?

Are there any cells filled out in this current sheet? The checkbox symbol will only appear once there's data in a row:
