Evaluate entire column for matching values

Hi!

I've tried a few ways but don't seem to be successful. I need to evaluate an entire column of data for the same values. So for instance, I need to evaluate Column A for all the same Agency Names. In instances where the Agency Name matches I need to then evaluate a 2nd column for matching titles. If both match.. input a unique identifier into the formula column.
