How do I use Index Collect formula?

I want to find the Initiative Name from the Project ID entered into the field (Initiative Story sheet) using Project ID, Company, and Priority Initiative from the source sheet (Company sheet) and if no initiative is found for any company leave it blank. How do I find the Initiative Name using the Index Collect formula?

@Tab Try using the Index Match formula instead

=INDEX({Initiative Name Range}, MATCH([Project ID], {Project ID Range}, 0))
