Hyperlink truncated when pulling into a report

Anyone know why this is happening? - this hyperlink is being pulled into a report via index/match, vlookup is doing the same, only a potion of the url is hyperlinked...why oh why? help!

@Anabisdally Castro instead of using a formula to pull the hyperlink have you tried creating a cell link or even possibly Data Mesh to populate the hyperlink?

@Garrett Henke, a cell link works but this would be a manual step, I am trying to auto populate this cell once a form is submitted with the info. Have not tried DataMesh, still would like to understand why the formula above is doing that... it has worked fine in the past, not sure what is going on...thanks for your help!
