checklist in form

I have a checklist I created in smartsheet, however, i want this checklist to be as a form as after each team complete the form, they need to submit

is there any way it can be done ?

Hi @zahrah

Yes! You can create a checklist as a Form in Smartsheet and have a submission create a new Row in a sheet.
