Notification Does Not Send

I have this sheet that is new.

Here are the settings for the form and for me:

This automation is not sending:

Not 4.jpg

I have also tried adding a date condition where it is Date is in the past, and the date is in the sheet as above, but that also does not send.

Any ideas?
