Forms with multiple lines

Hi,

Is it feasible to create a form where a user can submit 5 or less individual Risks/Issues with one Submit and these 5 or less get captured as rows one below the other on the underlying sheet.

On the settings for your form change it to this:

They will enter them one at a time and each time they submit it'll be a new row. It will keep reloading the form until they've added as many as they need to add.

This might help.

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

Would that work/help?
