Forms - Autopopulate fields based on name?

I am using a form to capture the names of attendees at a weekly event. Is there a way to set up the form so that it will autofill information based on the attendees last name?

Hi @ccisneros

Conditional Logic in forms will allow you to show certain fields/cells that these users can fill in, but it does not currently auto populate these fields with values. Do you need the form users to see this information, or is it for your own internal purposes?

If it's internal data, I would recommend using formulas in the grid of the sheet to automatically bring in values, or a change cell workflow.
