VLOOKUP with 2 conditions

Hi

I hope some one can help me :

I have 2 sheet one to enter data from form & other to record( update row )

i need formula to take start time from first sheet to update in second sheet wit tow condition ( name & date )??

Something I have done in the past is a little trick to make each record unique.

I create a helper column in both sheets with something like "=[Employee Name] + Date" then when you do the vlookup you lookup the unique value. This way there is only one record for the Employee and Date combination
