I can't find my form!

Created a new form and it looks great and other people can get to it. But it doesn't appear when I click Manage Forms and it doesn't show up in my Sheets list. Where did it go?

Does the sheet you're looking in populate?

Hi! That's the thing - I can't find the sheet it is attached to. I know it's the right form, because I recognize all the stuff I did. There is one from last year that shows up in Sheets, but it is considerably different. Don't worry about it - I just started over.
