@Miriam W. Hi, I suggest adding a requestor field (use a contact list column) to the form. Each individual can select their name from the drop-down menu of contact or enter theirs if not listed. When setting up the approval request workflow, in the last stage whether it's approved/declined select send to contact in cell which will be the requestor column.
