How do I properly add a file upload field to a form?
I keep trying to add a file upload field to a form, but it automatically disappears when I send out the form. Is there something I have to do in my sheet? Can someone please help?

Date format incorrect on forms
Since the updates to the forms, some of the forms we use now have the dates showing as mm/dd/yyyy rather than the format dd/mm/yyyy. Is anyone else having this issue, has anyone else found a way to resolve this? Thanks, John

Form has a contact list field, for the contacts are not pulling into the form
Hi, We created a form to take in change request. The sheet has a column 'Interested Party" and this is a contact list. When i am in the sheet, i can start typing a name and the contact list based onthe spell comes up. This does not work the same in the form. Is there a setting that I am missing?

How do I duplicate a form?
I would like a second form for my sheet that is very similar to the initial form I have created, but I do not see a way to duplicate the form, so I can make minor modifications. Do I need to recreate my current form from scratch? Or is there an easy way to duplicate a form? Thanks!
