Has anyone ever leveraged in a survey the ability to provide a list of options and have the survey user force rank the top 5? How would you set up the sheet/form to have them see the total list (15) and then force rank their top 5?

How do I make a form properly link to an image on a dashboard?
I have a created the form and linked it to several photos on the dashboard but when I click on the photo, instead of the poll view popping up - the backend summary sheet pops up. The goal is to have employees click on the item they would like and then fill out the survey that asks them their size.
