How do I make a form properly link to an image on a dashboard?

I have a created the form and linked it to several photos on the dashboard but when I click on the photo, instead of the poll view popping up - the backend summary sheet pops up.

The goal is to have employees click on the item they would like and then fill out the survey that asks them their size.

You would need to adjust the Widget Behavior for each photo. You would select "Open another site" and paste the appropriate form URL there.
