I am using a form to collect contact information for event registration. We want our guests to register separate email and phone numbers for each member of the household. Is there a way to create a conditional format that keeps guests from entering the same phone number and email more for more than one person?

Unfortunately, it's not possible in Smartsheet at the moment to use any conditional formatting or rules for forms, but it's an excellent idea!
Please submit an Enhancement Request when you have a moment.
