If you want to have it included in the form, you have to link it from somewhere.
You could also add the form and a sheet/report (with the attachment) side by side.
Would that work/help?

Re: How to populate a Smartsheet form dropdown
SBP has an add on tool that does this for you. Its called Smarter Dropdown List Manager. See here for more details.
Its in our lowest priced apps and is a great little tool. You can populate dropdown lists from source sheets or reports. The dropdown lists can be in the Grid or in the Sheet Summary, the app updates regularly so the lists are up to date.

Hi @Oden,
You can add a Comment field in the Approval workflow as shown in the screenshots below. The comment box will be displayed in the Approval Form for commenting.

Hi D, Hope this helps - I have shared a collection to my gmail (unlicenced account) and can see (via gmail access) what I need to see ? Also I have created a seperate workspace for the collections which is also shared to my gmail account.

Try this (replace Today@row with whatever row you have that has the submission date (Created@row, for example).
=WORKDAY(Today@row , 3)
