Can I split a sheet?

I have a form where I can assign a visitor badge, I also use the same form to return said badge. Am I able slit the sheet to one side is assigned and other side is return. For example, Joe Smith assigned a badge at 4pm and returned that badge at 5pm, the data was summitted by a form different times. Can I see this data in the same row? I hope this make sense. I am new to Smartsheet's Thank you

You would have to have something unique that would be used for both forms to match them together on the same row.

Do you have something that could be used to match the rows?
