Is there a form integration using smartsheet dropdowns?

We want to build a form for our website that collects data into smartsheet - we need to use some dropdown lists from a smartsheet table we have, this needs to be dynamic - we also want to add images and be creative with the form - does anyone know if this is possible with a 3rd party integration?

• For Dynamic Dropdown, I'd recommend the premium app, Data Shuttle. Is that an option?
• For the form, yes, there are 3rd-party options that be more "designed," but right now, I don't remember which ones you can add pictures to.
