Uncheck box when the value in the dropdown changes

As the title states, I'm trying to have boxes uncheck when the value in the dropdown changes. I have attached an image.

When "Stage" column changes clear "Fit Plan Attached", "Approval Sent", & "Approval" column check boxes.
