How can I create a formula
· If the DUE is within 5 days of TODAY, turn the health symbol YELLOW
· If the DUE DATE is in the past, turn the health symbol RED
· If DONE is checked, turn health symbol to GREEN
Would be so grateful for assistance!

Try something like this.
=IF(DONE@row = 1, "Green", IF(DUE@row <> "", IF(DUE@row < TODAY(), "Red", IF(DUE@row <= TODAY(5), "Yellow"))))
