Generate Documents in Automation will not autofill all information after mapping
Good Morning Smartsheets world. My Boss has recently been let go and I have taken over all of his smartsheets forms. When I have a customer submit a form it would go onto a smartsheets list. then I can generate a pdf document where it would take the customers information and prefill the pdf. Since the file transfers it will still allow me to generate the document but it will not autofill. I have looked into the mapping and it looked correct and I even tried making a new mapping and a whole new work flow but still no change. When it generates the document it only captures 3 items and not everything so the PDF shows to be incomplete.
Any thoughts on what I may be missing or overlooking. Thank you in advance.

Has anything changed on the sheet?
Did you add the fields to the form that works?

The Complaint, Cause and Correction are the only ones that fill in.
Has anything changed on the sheet?
Nothing has changed that I can see.
Did you add the fields to the form that works?
I added all the fields to the form and only the three C's work
