Allow form user to include commentary on approval workflow/automation

Hi all,
I'm creating an intake system for site related issues and want to allow the person assigned to the ticket to request approval of resolution before closing the issue. Ideally, the assigned owner would put a status of "Resolved - pending approval" that will trigger an automation to run an approval workflow. I need however to allow the form user/submitter of issue to put comments in (free text/number box) to provide more direction on why they might deny the resolution. How can i accomplish this? I am trying to cut down on the number of repeated forms completed (or issues logged) whenever tickets are closed that aren't fully resolved.

You could use an Update Request, and they can add a comment.

@Andrée Starå - that works great! I have it set to do an request update and include the status for them to input commentary. Do you have any suggestions of how to "remind" the requestor when they forget or delay?
