Form response saved in PDF format

Hi team,

I am looking for ways to save/ record the Form response in form of PDF. If user responds to the Form it going to record the response in PDF format of the same form with user information in it. Each response creates a pdf of the form with user data

Use case - we want to save it as an evidence that user signed the waiver in form format. Apart from the response recoded in a sheet which could be manipulated
