Form submission is auto declining.

I have a form set up with automation for the workflow. It has been working flawlessly until last week. Now every time a request is submitted it auto declines it right away without going to the 1st step in the workflow. This is a massive sheet with a lot of Information, and I don't want to start over from scratch. HELP!
