Drop-down list of forms

I am attempting allow users to select one smartsheet form from a list of forms. Each form in the list is a different survey. How to I setup a field (drop-down?) in one form so that a user can select and open one form of a number of forms?

You would have to set up an automation to send an alert to the user with a link to the form they have requested.
