Forms/ How to make columns into rows

I am looking to use forms to collect heavy equipment request for a large project. Each department will be submitting anywhere form 10-15 heavy equipment request at varying times and quantities.

How do i intake this information without each request being a column? Is there a way to automatically prepopulate it into a row? As every new feild is a column. I want to avoid the internal and external departments from having to submit the same form 10x to input equipment request. Is there a way or formula to automatically move the columns i would like into a new row? as one form submission is now 20 columns long at this point

This might help.

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.
