Hi - I am trying to write a form and would like to do the following:

1. Are you a member of XYZ?  If so, what is your membership number?

Is there an easy way to do it or do I have to have a separate column in the sheet and therefore another section on the form?

    If you just want them to be 2 questions then that's easy to do

     

    Are you a member of xyz?

    If yes, what is your membership number? _____

     

    The harder thing is if you are asking if there can be intelligence to only ask the second question if the first question is yes. As far as I can tell, that is not possible in smartsheet

    You could also do it as one question and just have the single membership number field in your sheet

    I agree with ricki's second post.

    In Smartsheet's Forms, you can customize the text displayed, so for a column name [Membership Number], you could display:

    Are you a member of XYZ?  If so, what is your membership number?

    as the header.

    If you need to do counts (how many members of XYZ do we have?) then either

    1. Use the check box method

    or

    2. Count total of non-blank member numbers using COUNTIF or COUNTIFS
