Data shuttle column mapping error

Hi All,
I created a data shuttle workflow to work on attachment - it worked perfectly fine until now. I have just attached a new file (which is exactly the same as previous ones, just with updated data) and the workflow does not run. When I try to edit the mapping it asks me to refresh it to see the most recent ones but then when I do it it seems not to recognize any of the excel file (attachment) column titles. The sheet has more that 12 columns that were previously mapped but now all it's showing me is one column. Not sure what happened.
Have you had such an issue before?

Does the file have multiple tabs? Is there a filter on in the excel file? Are there any new blank rows at the top of the excel file?
