2 forms within one sheet

How can I attach a form to an automation? I have 2 forms within my sheet. Form 1 is the main form that generates the lines into the sheet. The automation is an update request where they will need to fill out this form 2, but how do I make that possible in an automation? Once they do fill it out, where in the sheet will the info populate?

If the second automation is an update request, the information they fill in would populate in the corresponding cells within the row that triggered the update request.
