Manage Workflows (4) going to everyone instead of assigned approvers - help!

I manage a large training program of surgery residents and we use Smartsheet to manage the numerous requests we get for travel approval for conferences. This is the workflow we have that has worked fine up until about two weeks ago. Now when a resident submits a request, the request and response goes to ALL recipients in the four workflows instead of the recipients tagged to the correct program.
Anyone have any ideas?

Do you have filters in your workflow? If not, I would recommend you add in the filters and try running the automation (send only to yourself)
