This is the first time I've seen characters appear like this when building a form; I'm also using Chrome and I'm able to see text without any issues.

Is it possible you have a Browser Extension that could be interacting with the text on the form? Could you try turning off all extensions, then turning them on again one-at-a-time to see if this helps and if we can find the culprit?
