Dashboard Creation Using a Form

I have a form that I submit to stakeholders that captures CSAT data. The form specifically captures the CSAT score, the stakeholder product, the internal team that worked on it (e.g., Team A, Team B, and Team C), and the stakeholder's name. The date the form is submitted is also included and auto-populated in another column.

I would like to organize this data such that it populates a table similar to what is showing below:

The monthly cells would average the CSATs by team, per month. It would further provide the quarter average on the last row for all three teams.

Once that is done, I'd like to create a dashboard that visually displays this data.

Is that possible in Smartsheet?
