Reactivating a sheet/form

One of my sheets reached its limit which caught me off guard. Before I could even try and move or delete some rows my form that was connected with it deactivated and now I cannot even open it. Is there a way to reactivate the form?

Thank you I fixed the issue. I just created a template and started from scratch.
