Suggestions for multiple file uploads?

I'm looking for suggestions on the best way to create an intake form that would allow external users the ability to upload documents in our system. The idea is that internally we would be alerted when documents are uploaded, review and approve them. External users may need to upload 15 documents. Any suggestions?

You could add a form that reloads. Maybe something like this.

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

Would that work/help?
