I can only get 1 out of 5 automations to work. What am I doing wrong?

I have a Smartsheet with 5 automations on it.

1. When a line is added (working)
2. When the item is approved (3 different automations for 3 different fields
3. When the item is entered into our ERP

Only adding a line is working. Here is an example of one that is not working.

Untitled Workflow

Trigger:When rows are changed AND when 'All Courses Approved' changes to Any Value
Actions:Alert someone
Recipients:Louise Bradley

Any ideas on what I am doing wrong??
