Link to sheet is broken

Our team's orientees use Case Logs through Smartsheet to keep track of their progress while on orientation - after completing a certain task, they fill out a "case log" which includes several fields and then that case log is sent to their preceptor via email. When the preceptor gets the email, they click a link that brings them to the same case log where they (preceptor) then add their feedback. What I'm noticing is that the link that is sent to the preceptor leads to a form that is missing several fields that should be there. However, when I go to "manage forms" all the fields are there but for some reason they are not showing up when I click the email link. Help!
