Is it possible to utilize a template in control center that generates new form url in that sheet?

Is it possible to utilize a template in control center that generates a new form url every time the project toolkit is provisioned?

We are using a sheet (which is in this template) where the rows are populated via a form. If I use control center now it just overrides the template with every new project that's provisioned and the form is populated. Looking to see if its possible to generate a unique url for the form every time our project toolkit is provisioned.

If the form is built into a sheet that is part of the template, the new sheet provisioned by Control Center will have its own form which you can then grab the URL from.
