Help notes and header missing on the form

I have set up anew sheet with Help notes and a header, but after the first automation runs and you go back into the form, they are not showing anymore, how do i make them show on the form after the first automation? many thanks.

When you say " a new sheet with Help notes and a header" do you mean you added a row in your sheet that acts as a header row (this could include your help notes or they could be in a different row)?
If the above is what you're doing, what is your automation set to do? Taking a wild guess thinking your automation is moving the header row to a new sheet.
Should i be correct here, have you tried using the column description for your help notes?Marcé Holzhauzen
